Healthcare AI Governance Has Moved from Best Practice to an Operating Requirement

AI in healthcare is no longer just an innovation story.
For healthcare leaders, it has become a governance and accountability challenge.
Across North America, the direction is clear: AI is moving from experimentation to expectation—and that shift is now being reinforced through policy, regulation, and operational reality.
From Innovation to Accountability
Recent U.S. regulation reflects this shift.
ONC’s HTI-1 final rule links interoperability with algorithm transparency and introduces requirements for predictive decision support in certified health IT. These include expectations around validity and reliability, fairness and bias, transparency and intelligibility, safety, security, and privacy, governance over how data is acquired and used.
This changes the leadership question.
The issue is no longer whether AI tools can produce value. Many clearly can.
The issue is whether organizations can explain:
where AI is being used
what it relies on
how it is monitored
who is accountable when performance changes
This is not just compliance. It is an operating model requirement.
Lifecycle Oversight Is Becoming the Standard

AI is no longer treated as a one-time approval decision. Instead, it is managed across the full product lifecycle, with ongoing emphasis on post-market monitoring, transparency, bias, performance drift, and continuous validation.

The Real Risk: Fragmented Adoption
Many health systems are already using AI—but in fragmented ways:
point solutions in operations
embedded vendor functionality in EHR platforms
predictive models inside clinical workflows
generative AI layered into documentation and processes
The risk is not AI itself. The risk is using it without clear governance, defined ownership, consistent validation, workforce understanding.
That is where exposure grows—and where trust breaks down.
Governance as an Enabler (Not a Constraint)
The most important shift is cultural as much as technical. Effective AI governance is not a committee that appears after procurement. It is a discipline embedded across clinical leadership, digital and IT teams, operations, compliance and risk, and executive leadership.
Done well, governance accelerates adoption. It helps organizations distinguish between tools ready for scale, tools that require stronger controls, and tools that should not move forward.
What Leaders Should Be Asking Now
Leading organizations are starting with a clear set of questions:
Where is AI already present across the enterprise?
Which tools influence decisions or prioritization?
Who owns validation and ongoing monitoring?
What visibility exists in model inputs, outputs, and limitations?
If these answers are unclear, the governance gap already exists.

AI governance is no longer a marker of digital maturity. It is becoming an operating requirement for safe, scalable adoption. The organizations that move best will not be those that deploy the most AI. They will be the ones that can do so with:
